How to Register
Anybody living within the Practice area is welcome to join the practice. Acceptance on to the list is at the discretion of the Doctor. You will also be invited to attend for a health check with the healthcare support worker.
There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.
Please note that your medical records may take several weeks to arrive at the Practice.
We will accept patients living in Norton, Ball Green, Brown Edge, Endon, Stanley, Stockton Brook, Baddeley Green and parts of Bagnall and Milton.
Catchment area
We are happy to accept new patients who are ordinarily resident in the UK and living within our practice boundary.
See if you live within our practice boundary by using the interactive map and postcode checker.

For more information about NCRS please see: National Care Records Service
Information for patients who wish to opt out of having a summary care record, including an opt out form is available here: Opting out
Organ Donation
From 2020, adults in England will be considered potential donors unless they choose to opt out or are excluded. For more information please visit the Organ Donation NHS UK where you can also register your decision.
